This is to inform all the Computer Faculties and IT Assistants that they have to fill the ESI , EPF & Faculty  information form and bring alongwith them the following documents :

1. 3 Photographs passport size (self).
2. 2 photographs postcard size (Family).
3. Copy of joining letter.
4. Attendace copy of Feb & march month.
5. faculty information form is mandatory.
